- Deliver respite support for caregivers by positively engaging clients in meaningful one-on-one person-centred activities and provide companionship.
- Motivate clients through conversation, encouragement, and recreational activities.
- Implement individualized care plans with supportive interventions based on one’s personhood.
Personal Care Lead (PSW)

- Assist with personal care needs and activities of daily living.
- Assist with basic preparation of medications self-administered by the client.
- Perform light housekeeping duties, including laundry, meal prep, and assistance with feeding.
Care Coordinator (RN/RPN)

- Provide specialized care for clients such as, IV therapy, foot care, wound care, ostomy care, and basic tracheostomy care.
- Implementation of the nursing process in disease management and surveillance.
- Promote wellness through monitoring vital signs and physical assessments, and advocate for clients' safety at home.
- Support clients in end-of-life care and the implementation of comfort measures.
